Care is given
by a team of physicians, nurses, nursing assistants, social workers,
bereavement counselors, spiritual care coordinators and volunteers.

Physicians: Our physicians are board-certified and specialize in hospice and palliative care. They work closely with the patient’s own doctor, and are available for consultation whenever needed.
Registered nurses: Our registered nurses are experienced in pain and symptom management. They oversee the plan of care and serve as the point of contact and coordinator of services for the patient, the family and the HHCRI team. They make regular visits to monitor your condition, manage medications, provide injections, and answer medical questions. They also teach family members how to care for you.
Nursing assistants: Our nursing assistants offer help with everyday activities such as bathing, feeding, dressing, hair and nail care, etc.
Social workers: Our social workers are assigned to each patient and family to offer emotional support, to counsel patients and family members, to help with financial and household concerns. They provide help with advance care planning such as completing advance directives and funeral arrangements and make referrals for other support services.
Bereavement counselors: Our bereavement counselors ensure that the family does not feel abandoned after a loved one has passed away. By maintaining close contact with significant others for up to a year, our bereavement counselors help you grieve in the way that is natural to you, through support group sessions and individual counseling.
Spiritual coordinators: Our Interfaith spiritual care
coordinators offer emotional and spiritual comfort and support to
patients and families of all faiths and spiritual traditions.
Through a compassionate presence they honor the meaning and purpose
of a patient's life and assist each patient in moving toward divine
peace.
Volunteers: Our volunteers provide practical and emotional support to our patients and families by helping with patient care, grief or spiritual support, community outreach, office support, fundraising, or through our complementary therapies programs.
