A full range of
Hospice services are available to patients in Long Term Care facilities.
Under Medicare
regulations, patients residing in Nursing Homes are entitled to
the same benefits and services as patients living at home.
Hospice services in a Long-Term Care facility are an "added-on-benefit"
to the care the nursing home provides. Nursing home
and hospice staff, along with the patient and family, work
together to develop a plan of care promoting the best quality of
life, according to that person's wishes, goals, and values.
n a
regular basis, a member of the hospice team visits the facility
to:
- Coordinate care and support the facility’s clinical efforts
- Provide social work and spiritual coordinator visits as needed
- Assist with the personal care of residents
- Provide companionship from a volunteer
- Provide grief counseling to staff and family members
Medications, supplies and equipment related
to the life-threatening illness are covered under the Hospice Benefit.
