Directions to Main Office, 169 George Street, Pawtucket, RI 02860
From the South: Take Rt. 95 North to Exit 27 - Downtown Pawtucket. At the third traffic, turn right onto George Street. Continue straight through the traffic light, past the school. Immediately past the school, right before the split in the road, on the right side, you will see the entrance to Home & Hospice Care of Rhode Island. It's a large, brick building. Parking is available in the Visitors' spots or in the back of the building.
From the North: Take Rt. 95 South to Exit 27 - Downtown Pawtucket. Turn left at the end of the ramp. Continue through two traffic lights, past a school. Immediately past the school, right before the split in the road, on the right side, you will see the entrance to Home & Hospice Care of Rhode Island. It's a large, brick building. Parking is available in the Visitors' spots or in the back of the building.
Directions to the Philip Hulitar Inpatient Center, 50 Maude Street, Providence, RI 02908
From the South: Take Route 95 North to Exit 23 - stay right on the exit ramp going toward "State Offices". At the end of the exit ramp, turn right onto Orms Street. At the 2nd set of lights, turn right onto Smith Street and continue to the Newport Creamery. Turn left into the Roger Williams Elmhurst Campus. The Hulitar Center is ahead on your left.
From the North: Take Route 95 South to Exit 21 - Atwells Avenue exit. Turn right onto Atwells Avenue. At the next set of lights, turn right onto Dean Street. At the 7th set of lights, turn left onto Smith Street. Follow to Newport Creamery, turn left into the Roger Williams Elmhurst Campus. The Hulitar Center is ahead on your left.
Directions to the Wakefield Office, 143 Main Street, Wakefield, RI 02879
From the North: Take Route 95 South. Merge onto RI - 4 South via exit 9. Turn slight right onto US-1 South. Continue to follow Route 1. Take the ramp toward Wakefield. Turn right onto Old Tower Hill Rd. Old Tower Hill Rd. becomes Main Street. End at 143 Main St.
From Westerly, RI and beach areas: Take US-1. Continue north on US-1. Take Salt Pond Road ramp toward South County Hospital. Turn left onto Salt Pond Rd. Turn left onto Woodruff Ave. Turn right onto Main St. End at 143 Main St.
From Connecticut: Take Route 95 North toward Providence. Merge onto RI-138 East via exit 3A toward Kingston/Newport. Turn slight left onto RI-138. Continue to follow RI-138. Turn right onto RI - 108. Turn left to stay on RI-108. Turn right onto Old Tower Road. Old Tower Road turns into Main Street. End at 143 Main Street.
