Administrative Assistant

Provides timely, responsive, customer-focused and customer-friendly service to all patients, families, staff, students, vendors and visitors to HHCRI and specifically, the Wakefield office. Provides administrative support and assistance to the HHCRI South Team in the coordination and support of patient care delivery. The Administrative Assistant is a liaison between the patient, the care giving staff, other internal/external departments and the billing department. Provides comprehensive chart management and communications between all parties to ensure seamless care.
One to three years experience and/or training or equivalent combination of education and experience.
Help Center Referral Coordinator
Full Time, Monday-Friday 8:00-4:30pm
Provides timely, responsive customer focused and customer friendly service to all referral sources, patients, families, staff, students, vendors and visitors to HHCRI. Acts as a liaison for customer calls to coordinate visits with the RN Liaisons, Hospice Representative and/or Admissions RN. The Help Center Referral Coordinator reports to the Help Center & Admissions Manager and in his/her absence, reports to the designated manager, the Director or Administrator On-Call (AOC).
Must be able to read, write and speak English to the degree that allows them to perform all aspects of their job proficiently. Ability to read, analyze, and interpret common reports and documents. Ability to communicate and respond to common inquiries or complaints from patients, family members, co-workers and management.
Must have 1+ years experience in customer service or sales. Must have strong active listening skills and verbal/written communication skills. Must be able to work independently and meet with families in a variety of locations, including homes, hospitals, and nursing homes. Must be able to travel the state of Rhode Island.
Valid Driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.

The Hospice Aide (HA) is a member of the interdisciplinary team under the direction and supervision of a registered nurse or therapist. Performs various services for a patient as indicated on the plan of care to meet the patient’s personal needs and to promote comfort.
Graduation from an approved educational program. Current certification for practice in the State of Rhode Island as a nursing assistant. At least one year of home care and/or hospice experience.
Manager of Counseling Services
Full-time, Monday-Friday 8:00am-4:30pm
Location: Providence, RI
The Manager of Counseling Services oversees and evaluates counseling services provided to meet the needs of hospice patients and their families through the clinical supervision and development of counseling services staff of approximately 15 social workers and 10 chaplains.
Reports directly to the V.P. of Patient Care Services, serves as the clinical expert in Counseling Services and works directly with the Dir. of Patient Care Services and the Patient Care Managers to ensure that the highest practice standard in each counseling discipline is maintained.
Master’s Degree in Social Work from an accredited school. Rhode Island LICSW (Licensed Independent Clinical Social Worker) or LICSW eligible. 2-5 Years community resource experience. 3-5 years clinical counseling experience in hospice or health related field. Experience in pastoral care preferred. Minimum of 3 years management and supervisory experience.
Medical Social Work
Per-Diem Opening: Commitment of 6 day shifts per month, including 1 weekend shift per month
Location: Long Term Care C/Central Region Homes (RI)
Responsible for the implementation of standards of care for Medical Social Work Services and ongoing Patient Care Services. Medical Social Services are provided by a qualified social worker (MSW) in accordance with a plan of care.
Masters Degree in Social Work from an accredited school. State registered independent social worker (LCSW or LICSW in RI) preferred. LCSW accepted while working toward LICSW. Two to five years of home care and/or hospice experience preferred. Gerontology experience preferred. Valid driver’s license with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
RN – Per Diem Flex
Commitment of 6 shifts per month during weekdays and on-call per diem shifts 3:00pm-11:30pm and/or 11:30pm-8:00am

The RN Case Manager has responsibility to ensure that every patient’s individualized plan of care is developed, revised, and maintained throughout length of stay.

Spiritual Care Coordinator
Full-time, Monday-Friday, 8:00am-4:30pm
Location: Providence
The Spiritual Care Coordinator actively participates as part of an interdisciplinary team to coordinate and provide spiritual care to patients, families and staff, while accepting, affirming and respecting diverse belief systems and spiritual practices.
Masters Degree in religious or theological studies required. Minimum of 2 units of Clinical Pastoral Education or equivalent training, education and experience. Two to five years of experience in community service. Must be comfortable with prayer in an interfaith environment and respectful of all religious beliefs/spiritualities. Knowledge and experience with grief and end of life issues preferred. Knowledge of caring for patients and families in the home setting or inpatient facilities preferred.
