Anyone can make the first call to Home & Hospice Care of Rhode Island. It may be you, a family member, a good friend, or a member of the clergy. The call does not have to come from your physician or a health-care provider. The number is 401-782-0725 or toll-free,
1-800-338-6555.
HHCRI will contact the individual's physician and an admission nurse will schedule a visit to develop a plan of care.
For physicians
Home & Hospice Care of Rhode Island has six highly qualified and experienced medical directors and three registered nurse practitioners to provide education and input into a plan of care. HHCRI's Help Center will coordinate your call. It is available 24 hours a day, 7 days a week.
